Please plan to come to your visit 15 minutes before the scheduled appointment to allow time for required processing which includes:

1. Registration  - You may download a copy of our registration forms so they may be filled out prior to your appointment.  This includes personal and family history as well as insurance information. Click here to download registration forms 
   


2. Insurance is checked prior to every visit and updates are added to your medical file.
   


3. Medication lists, weight, height and blood pressure are checked when appropriate by our medical staff along with the reason for your visit.
   


4. History of your current concerns are taken by your medical provider.
   


5. Physical Exam to aid in the diagnosis of your concerns along with any diagnostic tests.
   


6. Diagnosis and treatment plan are developed and you are instructed as to the best course of treatment for your concerns.
   


7. Follow-up or Referral to an allied healthcare professional may be arranged in order to optimize your healthcare.
8. Call to become web enabled so that you may access our patient portal and update all your information electronically.
